Introduction to In-home Care

Any patient that is diagnosed with an illness that has lasting effects can benefit from In-home Care. In-home Care, or In-home Support, is the provision of support services to patients with all levels of ability or disability in their home or wherever they reside. Whatever services a patient requires, they can be provided as an In-home Support to maximise health outcomes.

Maple Community Services can provide all of their support services in an In-home setting when needed, including variable levels of in-home and out of the home support.

In-home Support Information

Whether a person suffers from a physical disability, a psychosocial disability, or a combination of the two, they may experience challenges leaving their home or their place of residence. The point of In-home Support is that patients don’t have to take on any added burden to their illness by having to leave their home when it is not feasible, not advised, or not in their best interest. In-home Supports span the range of services that are provided for people living with disabilities, including everything from help with day to day tasks to complex therapeutic measures.

By incorporating a multidisciplinary approach, specialists evaluate a patient’s ongoing needs in multiple functional areas, as well as the continuing impact of their chronic illness. This informs the need for In-home Supports and can improve a patient’s ability to function more independently, ultimately removing the need for In-home Care.

We have four major categories of In-home Support Services. Each type of support service contributes to your overall well-being and expands personal options to live a life that is most fulfilling.

Core Supports

Supports that enable In-home Support patients to complete activities of daily living, working towards an individual’s goals, and meeting their set objectives.

Support Coordination

We help identify, coordinate, and access support services for In-home Care and disability service providers on your or your loved one’s behalf.

Plan Management

Maple Community Services manages the funding of your NDIS Plan. We are the go-between for you or your loved one, providers, and the NDIA.

Accommodation

If needed, Maple provides supported independent living with accommodation assistance and/or supervision of daily tasks to help you live as independently as possible. This goes hand-in-hand with our In-home Support services as we can provide the precise level of care and support required.

Types of In-home Care Support Services

Our team works one on one with you to help you with day-to-day activities, and connects you with high quality treatment and therapy services. Our supports includes a full range of different services for you to determine which will best suit you. Options include any or all of the following:

Personal Care

Maple Support Specialists assist people requiring In-home Care by helping them live independently. Whether you or your loved one is in a group home or in a private residence, we show up when we’re needed to help with morning routines, personal hygiene, other daily tasks including eating and drinking, and assistance during appointments.

Household Tasks

At Maple, we understand the value of healthy eating and keeping a clean home, particularly if you are homebound or require assistive services in your home. As a part of our NDIS services, we provide help with household tasks like cleaning, meal preparation, banking, shopping, and so much more.

Travel and Transport

Maple Community Services is here to support you or your loved one by assisting with transport to and from wherever you need to go. While we provide In-home Support, if a patient also has transport needs, we can provide transportation to appointments, outings, shopping, religious places of worship, and for students, to and from school.

Garden Maintenance

If you or your loved one are unable to facilitate gardening tasks, the NDIS provides funding for this service. Patients requiring In-home Care may not have the capacity to spend the time and energy tending to a garden. At Maple, we’ll do the gardening for you, allowing you or your loved one to maintain a healthy garden and keep your lawn tidy.

Community Participation

Maple Community Services can assist you or your loved one in accessing and participating in the community. Individuals requiring In-home Care may find this particularly challenging. That’s why we will guide, support, mentor, and motivate you or your loved one to succeed and reach their goals for activities, social interaction, and recreation.

Life Skills Development

Developing Life Skills is about building confidence to enable independent living. We do this by developing a plan which gives you the training you need to achieve your short- and long-term goals. The support we provide includes education on personal hygiene, maintaining a clean environment at home, organisational skills, and social interactions to name a few.

Coordination with Professionals

If you wish to seek therapy, counselling, or other professional insights to help you manage your disability requiring In-home Care, Maple can assist and match you to highly skilled specialists who will provide you the professional support you need.

Customized Home Services

We work with clients to assess their functional and therapeutic needs to achieve a healthy and fulfilling life wherever they are. In the comfort of your own home, and with specialised In-home Care support services based on cultural and language needs, you can learn to live more independently.

Is In-home Care Covered by the NDIS?

The National Disability Insurance Scheme (NDIS) is available throughout Australia to anyone under the age of 65 who can demonstrate they have a permanent intellectual, physical, sensory, cognitive, or psychosocial disability. The overall aim of the NDIS is to improve a person’s ability to function in all aspects of daily living and, over time, gain more independence.

If you or your loved one has one or more of these disabilities requiring In-home Support from professional caregivers or support specialists to manage their disability, you may be eligible for support under this scheme.

At Maple Community Services, we can walk you through the assessment process to see if you qualify for NDIS In-home Support benefits. If you or your loved one meets the criteria, we can help facilitate the process and have the full capacity to provide the comprehensive support services you need.

What Can NDIS Fund For In-home Care?

The NDIS aims to provide access to high quality, personalised, and innovative supports and services to enhance the independence and social and economic participation of people with disabilities and their carers.

The NDIS funds several critical services for people who require In-home Care. These include personal care support, community access support, nutritional support, skill development, transport, household tasks, and behavioural support. The types and amounts of funding available are partly determined by the severity of a person’s disability and resulting impaired function.

By partnering with Maple Community Services, together, we can help you identify your priorities and cater a support plan that provides the specific services you need, enabling you to lead a fulfilling life.
